At your initial appointment Dr. Baranovsky will do a complete examination of your teeth and gum tissues. The staff will assist in gathering necessary diagnostic tools to allow Dr. Baranovsky to formulate the best possible treatment to achieve optimum dental health. At a second appointment we will present a written report for you to take home and review, detailing your treatment necessities as well as any options that may be available. Occasionally, there is a third appointment to narrow down the options or to assist in additional treatment with complicated treatment plans.
Prior to your first appointment, please, complete your forms online. You will be taken to our confirmation partners’ website to complete and electronically sign your demographics, insurance information, health and dental history, HIPPA consent, photography consent, office policies forms.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit. Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication
(i.e., heart medications, aspirin, anticoagulant therapy, etc.).
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office for your consultation. If additional films are necessary, they will be taken at our facility.
